I am a dual Bachelor’s prepared (Mental Health and Nursing) Registered Nurse/ Certified Case Manager; currently enrolled in a Master’s degree program. My past experience includes: direct patient care, Case Management and Care Coordination. I am available to assist adult patients with their medication management, monitoring their blood pressure/vitals, blood sugar, educating them on managing their self-care at home, coordinating and keeping up with their appointments as well as accompanying them on appointments. My rates for in-home Case Management services are $65/hr. Should accompaniment to an appointment be required for a particular day, the rate is $75/hr, which includes personal transport to and from the appointment.
