Access & Assistance information and referral assistance. Assist client with their public and private benefits throughout 12 assigned-counties (ex. Medicare Part A, B, & D benefits/ Medicaid (dual eligibility) and other state government benefits). Conducted home visits when needed, documents care plan (ADL 2060 assessment) progress, and evaluate each plan and modify best option available, and continuation outcomes, with follow-up and if other services needed. (Ex. Respite Care, Emergency Response Services, Home-Delivered Meals). Conducted an evidenced- based fall prevention program, which has helped assist clients in decreasing falls and increased physical activities. Upon a completion assessment of the patient's needs, they routinely set goals for patients - reduction in AIC levels for diabetic patients. These goals are incorporated to become the criteria against which results can later be measured to determine specific outcomes. As a case manager for evidence based programs we provide older adults with the tools to maintain their health, reduce their risk of developing chronic diseases, and manage their health to live as independently as possible. Certified volunteer, who advocates for residents and quality care in Texas Nursing facilities also help complaints by residents and staff. A professional staff Ombudsman closely supervises the volunteers and provides information to residents and families about rights and ongoing procedures of care for residents of these facilities.
