Client is being discharge from a weeklong hospitalization for diverticulitis and will need around the clock care at home for duties such as bathing, dressing, assistance with toileting, bring in the mail, taking the trash can to the curb, for possibly 30-60 days at least. The assisted support may become a permanent need to allow client to remain in home if client cannot gain back full independence. Client is fully alert and able to communicate clearly as to her needs.
