Need Help with Mom

My mother is 77 years old and has had an extensive hospital stay from January to May. When she started her hospital stay her mentation was "normal."
Over the months being in the hospital and having a small bowel surgery she has become paranoid with some hallucinations. She has been refusing medications and food. She has been recommended for memory care at this time.
She may refuse care but I want to ensure her safety. They say she may regain some sense of normalcy and being at home could possibly help.
She does have some doctors appointments that she would need a ride to in Pollocksville, NC.
Help suggesting she take her meds, checking her blood glucose, getting dressed and even eating. Some light house keeping but keeping her safe is the most important. Mom likes to go to the grocery store, book store and getting her hair done. He favorite thing is reading and just watching movies, basketball, and football games.
I would need someone Monday through Friday or it may only be 4 days a week until I can adjust my schedule, from 8AM to 5:30 PM.
