Care for mother with mobility and cognitive issues

My mother lives at home by herself. She is no longer able to drive and has mobility issues. In particular, she is not able to walk around without assistance and is prone to falling if she does not have assistance. She has been diagnosed with white matter disease dementia. She needs someone to come over for a few hours a day to check on her, help her get groceries, and run other quick errands. Preferably, the individual would come over for a few hours at least 3 to 4 times a week.
