Hospital Discarge due to confusion and seizures

My sister has experienced seizures since childhood . Shes 42 and was recently discharged with seizures that are happening when she slept, so severely that shes struggling with mental alertness and confusion during the day when shes awake.
she is under treatment- meds, PT, home health. Requesting:
local transportation assistance
Laundry assistance
Meal prep
light cleaning
