Elder care assistance

My mother recently got back home after a 3 month hospital + rehab stay. She still has very limited mobility and needs help to get out of bed, into and out of a wheelchair, to go to the bathroom, etc. I would like to have someone come in and help her move around and use the bathroom, help wash her, etc.
