Caregiver for 13 yr old with intellectual disabilities and seizure disorder

Looking for a special needs caregiver for my 13 yr old daughter for 3 days a week on Thursday, Friday, and Saturday or Sunday approximately 12-16 hrs per week to help with feeding, prepare medication, monitor safety, interact with her to play and take her for a walk with stroller or to park to play.
