Ederly Care mobility assistance and supervision

Looking for day care for my mom. She is recovering from an ischemic stroke and need mobility assistance (walker and wheelchair), medication reminder, meal prep, glucose testing, blood pressure monitor and 100% supervision. Diabetic and Hypertensive. Spanish speaking a plus.
