After School Care Needed for Kindergarten 5 yr old CP in WC

Granddaughter has cerebral palsy and needs assistance with transfers, positioning and ADLs. She's verbal and active but needs help with movements. She's in a wheelchair and we need someone to assist with afterschool care.
Need to live within 15 minute drive from Jupiter & 190/75.
