Caregiver(s) needed for two separate shifts, 6a - 9a then 3p-7p

Family is looking for a caregiver(s) for two separate shifts. Caregiver(s) must have their own care for errands and extracurricular drop-offs/pick-ups. Care will be provided to 13-year-old child to support with before and after school care needs. Continued support in morning and afternoon shift will be provided to parent and household management tasks. Caregiver must be willing to follow child's parenting and reinforcement plan (This will be provided by parents). Caregiver must also have experience with caring for teens (12-14) with ADHD.
1st shift 6a–9a: Preparing breakfast, packing a healthy lunch for child and parent. Caregiver will support child's morning routine and ensure on-time arrival at school bus stop. After, household cleanup including breakfast/lunch prep, grocery list management, shopping or pickup, errands and pharmacy pickup/drop off when needed.
2nd shift 3p–7p: Supporting child with homework (aiding and checking completion, sharing any academic issues or problems that arise during homework help). Ensuring snacks are prepared and kitchen is cleaned. Driving child to extracurricular programs/activities. Household clean-up, grocery list management, shopping or pickup, errands and pharmacy pickup/drop off when needed.
Ideal candidate must be a reliable and a non-smoker, with a calm and warm demeanor. Experience in supporting executive-function routines (checklist, reminders) for children ages 12-14 is required. A valid driver's license is required as well, please note Background Check and Motor Vehicle Check will be processed through Sittercity prior to care starting.
Pay rate is $20-$30/hour and family will cover mileage costs. Start date is needed ASAP (2-week paid trial period) and the position will last for 3 months, with the potential for ongoing care.
