We are seeking a reliable, patient, and compassionate Home Care Assistant to support an 59YO male recovering from recent medical complications. He is currently experiencing cognitive confusion, limited mobility, and recent hospitalization requiring ongoing monitoring and daily assistance. The ideal candidate will provide steady, calm support while helping him maintain comfort, dignity, and routine throughout the day.

Primary Responsibilities Include:
- Administer or supervise daily medications as directed by family/medical team
- Monitor cognitive status, behavior changes, and report concerns promptly
- Provide meal preparation and ensure adequate food and fluid intake
- Drive patient to medical appointments/therapies and regular errands (Pharmacy pick ups or grocery shopping) when required
- Maintain a clean, safe, and organized home environment
- Assist with light housekeeping and laundry as needed
- Offer companionship, conversation, and emotional support
- Coordinate with family and healthcare providers on updates and needs

Qualifications:
Prior experience as a caregiver, home health aide, or CNA preferred
Experience working with individuals with cognitive impairment
Ability to assist with mobility and perform physical tasks safely
Reliable transportation
Strong communication skills and patience
