Role Overview
We are seeking a proactive health professional to provide high-level ADL and cognitive support for a 37-year-old mother of two recovering from a cerebellar hemorrhage and splenial stroke. She also has type one diabetes and Celiac.
We are looking for an aide who can provide rehabilitation support in the home environment to support recovery between outpatient and home health care PT/OT/Speech.
Basic duties would include daily assistance with mobility/safety, ADLs, cognition work, and implementing practice of outpatient PT/OT/Speech strategies and plan of care.
Duties would be in home, but would also include short trips into community for experiences outside the home. Aide would have access to client's car.
