Senior care for post-hospital discharge

I'm looking for temporary care for my mother (age 76) who was discharged from the hospital this past weekend. She is in pain and has limited mobility and needs help with daily activities - changing linens, laundry, making meal, helping her with in-home tasks.
This is for a limited duration.
